A top-down action roguelike in which Isaac, a young boy, flees to the basement of his house after his mother, a religious fanatic, tries to sacrifice him. You make your way through dungeons that are randomly generated on every run, shooting tears at grotesque-looking monsters and bosses. Along the way you pick up items that change your attacks and stats. If you die, you start from scratch; this version includes Rebirth and its expansions Afterbirth and Afterbirth+.
